PROGRAMMING CREDITS
Requires qualifying programming. Receive a credit for each of the first 12 months after service activation,
as follows: $10 per month with America’s Top 250, America’s Top 200, America’s Top 120 Plus, America’s Top 120, DishLATINO Dos;
$13 per month with DishLATINO Max; $7 per month with DishLATINO Plus; $5 per month with DishLATINO Clásico. When you create an
online account at www.mydish.com/getonlinebonus, receive an Online Bonus credit for each of the first 12 months after offer
redemption, as follows: $25 credit per month with America’s Top 250; $20 credit per month with America’s Top 120 Plus, America’s
Top 200; $15 credit per month with America’s Top 120; $10 credit per month with Smart Pack; $9 per month credit with DishLATINO Dos,
DishLATINO Max; $6 per month credit with DishLATINO Clásico, DishLATINO Plus. You must redeem Online Bonus credit no later than 45
days from DISH service activation. You must maintain qualifying programming to receive all credits. You will forfeit credit in the
case of a downgrade from qualifying programming or service disconnection prior to 12 months of credits. After you have received all
credits, then-current price will apply.
HD FREE FOR LIFE
Requires qualifying programming. $10/mo HD fee waived for life of current account, limited to channels associated
with selected programming package. Qualifying programming packages are America’s Top 120, America’s Top 120 Plus, America’s Top 200,
America’s Top 250, DishLATINO Plus, DishLATINO Dos, and DishLATINO Max. You may forfeit free HD in the case of service disconnection.
PREMIUMS FREE FOR 3 MONTHS
Receive a credit equal to the monthly cost of HBO, Cinemax, Showtime and Starz for each of the first 3 months after service activation
unless you elect to downgrade one or more movie packages prior to the end of your promotional period. Offer value is $135. After 3
months, then-current price will apply unless you elect to downgrade. If you downgrade within 30 days following end of 3-month period
there will not be a Change of Service Fee.
BLOCKBUSTER @HOME FREE FOR 3 MONTHS
Receive Blockbuster @Home (regularly $10/mo for 1 disc at a time) for free for the first 3 months after service activation.
After 3 months, then-current price will apply unless you elect to downgrade. If you downgrade within 30 days following end
of 3-month period there will not be a Change of Service Fee. Requires the following: online DISH account for discs by mail;
broadband Internet to stream content; HD DVR to stream to TV; HD equipment to receive full range of channels. You can exchange
online rentals for free in-store movie rentals at participating Blockbuster stores.
DIGITAL HOME ADVANTAGE EQUIPMENT:
All equipment remains the property of DISH at all times and must be returned to DISH within thirty days of account
deactivation or you will be charged an unreturned equipment fee ranging from $100 to $400 per receiver. Lease upgrade fees are
not deposits and are non-refundable. You will be charged a monthly equipment rental fee for each receiver beyond the first, based
on model of receiver. WHOLE-HOME HD DVR: First Hopper with Sling HD DVR receiver and up to 3 Joey receivers available for one-time
$99 Upgrade fee. $199 Upgrade fee waived with subscription to America’s Top 120, America’s Top 120 Plus, Familytime Favorites
bundle, America’s Top 200, America’s Top 250, America’s “Everything” Pak, DishLATINO Plus, DishLATINO Dos, or DishLATINO Max at
time of service activation. A second Hopper with Sling HD DVR receiver is available for a one-time $199 Upgrade fee. With a second
Hopper with Sling HD DVR receiver, one additional Joey receiver is available for a one-time $99 Upgrade fee. Hopper and Joey receivers
require an eligible HD package and cannot be combined with any other receiver models or types. PrimeTime Anytime and AutoHop features
must be enabled by customer and are subject to availability. PrimeTime Anytime is not available in all markets. AutoHop feature is only
available with playback of select primetime shows on ABC, CBS, FOX and NBC as part of PrimeTime Anytime feature. Recording capacity varies;
2000 hours based on SD programming. Equipment comparison based on equipment available from major TV providers as of 1/17/13. Some features
require compatible mobile device. ALL OTHER RECEIVER MODELS: No more than 6 leased tuners/3 leased receivers per account are permitted,
and a lease upgrade fee will apply for select receivers, based on model and number of receivers.
Digital Home Advantage offer is available from DISH and participating retailers for new and qualified former DISH residential
customers (only 1 per household) in the continental United States. If qualifying service is terminated prior to end of 24 months,
a cancellation fee equal to the lesser of $480 or $20 per month for each cancelled month of service will apply. You must provide
your Social Security Number and a valid major credit card. Participating retailers may require additional terms and conditions.
The first month of DISH service must be paid at time of activation. Number of channels may decline. Local and state sales taxes
may apply. State reimbursement charges may apply. Where applicable, monthly equipment rental fees and programming are taxed separately.
For this offer, Standard Professional Installation includes typical installation of one single-dish antenna configuration,
typical hook-up of eligible receiver configuration to up to 6 TVs and equipment testing. More complex installations may require
additional fees; other installation restrictions apply. Prices valid at time of activation only; additional fees will apply to
upgrade after installation. Any unreturned equipment fees will automatically be charged to your DISH account or credit or debit
card provided to DISH. DISH shall determine eligibility for this offer in its sole and absolute discretion. Programming and other
